All new antibiotic orders will be identified and an Antibiotic Stewardship User Defined Assessment (UDA) will be generated, allowing our consultant pharmacist to identify drug-drug interactions, appropriate dose, and any relevant lab needed. This UDA will then be reviewed and completed by preventionist before being routed to the physician and DON for completion.
